Participate Assistance And Care (PAC)
PAC services enable individuals with intellectual or developmental disabilities to live successfully in their own homes, participate in their communities, and avoid institutionalization. These services provide hands-on assistance, prompting, monitoring, and support for daily living activities to ensure health, safety, and well-being.
Key Features of PAC Services:

Assistance with personal care, meal preparation, shopping, errands, scheduling appointments, and leisure activities (excluding transportation).

Support for mobility, including transfers, ambulation, and the use of assistive devices.

Help with correspondence, bill-paying, and other administrative tasks.

Escorting individuals to community activities and appointments.

Monitoring, reinforcement of behavioral supports, and adherence to risk plans.
Available individually or as a shared service, PAC focuses on supporting independence while addressing the unique needs of each individual.

Touch of Hope understands that all individuals should be treated with the same respect and dignity and given opportunities as everyone else in our society. People with intellectual/developmental disabilities are not always given opportunities like people without disabilities.

Transportation - Errands
Our caregivers provide assistance with transportation and running errands for loved ones who may have mobility limitations or other challenges. Caregivers offer reliable transportation to Appointments, grocery stores, or other destinations.
Additionally, we help with tasks such as picking up prescriptions or shopping for essential items. The goal is to enhance accessibility to necessary services and activities, promoting independence and convenience.

Light Housekeeping
Our loving caregivers help with minimal household chores and cleaning tasks to maintain a clean and organized living environment. This may include activities such as dusting, vacuuming, sweeping, and light tidying up.
The goal is to create a comfortable and clean home environment for loved ones who may require additional support with daily household responsibilities

Meal Prep & Feeding Assistance
Our knowledgeable caregivers assist with the preparation of nutritious meals. Feeding assistance may include helping loved ones with limited mobility or other challenges in safely consuming their meals. This ensures loved ones receive proper nutrition for their well-being.

Companionship
Our caregiver's primary focus for your loved ones are providing emotional support, social interaction, and a familiar friendly presence. Caregivers engage in conversations, recreational activities, and shared interests to alleviate loneliness and enhance the overall excitement for daily activities.

Grooming & Hygiene Assistance
Our loving caregivers provide support with personal grooming and hygiene tasks, ensuring individuals receive assistance in maintaining cleanliness and personal care. It may include activities such as bathing, dressing, and grooming to enhance overall well-being.

Functional Transfers
Our certified caregivers assist with physical movements and transfers, helping loved ones safely navigate activities like standing up, sitting down, and moving between different surfaces. This enhances mobility and independence in daily activities.

Vital Signs Performance
Our caregivers monitor and record important vital signs such as heart rate, blood pressure, and temperature. Ensuring that regular tracking of these important key indicators are crucial for assessing the overall health of your loved ones and aids in early detection of potential health issues.

Medication Reminders
Caregivers provide Medication reminders that timely prompts your loved ones to take their prescribed medications. This service helps ensure adherence to medication schedules, promoting effective treatment and management of health conditions.

Accompany to Doctor Visits &
Appointments
Our caregivers can happily accompany your loved ones to doctor visits and appointments this added support helps navigating healthcare services.
This may include transportation to medical facilities, assistance with paperwork, and ensuring a comfortable and reassuring presence during medical consultations. This service aims to enhance accessibility to healthcare and promote overall well-being.

Traci Alexander MSW, LSW is a licensed social worker with a concentration working in the field of mental health and addictions. Traci Alexander has years of experience working with individuals with developmental disabilities as a Direct Support Provider. Additionally, Traci has years working with families and children as a care coordinator and mental health counselor. Traci is committed to helping individuals to reach their full potential by instilling hope.

Community Based Day Habilitation (Individual & Group)
Community Day Habilitation services focus on helping individuals with intellectual or developmental disabilities acquire and retain skills in areas such as self-care, socialization, daily living, communication, and community living. These services aim to foster independence, build relationships, and create natural supports.
Key Features of Day Habilitation Services:

Activities are offered in community settings or non-residential facilities approved by FSSA/DDRS, separate from the individual’s home.

Training and support for engaging in leisure activities, community/public events, educational programs, hobbies, and unpaid work experiences like volunteering.

Assistance in maintaining relationships with family and friends.

Education in self-direction, including developing self-advocacy skills, exercising civil rights, and taking responsibility for services and supports.

Skill-building to promote greater independence, integration, and productivity in the community.
Day Habilitation is customized based on the Person-Centered Individualized Support Plan (PCISP) to help individuals achieve their personal goals and enhance their quality of life.
